Keep training evidence visible before a task repeats.

Use one record to keep an approved source, safe sample reference, reviewer finding, held action, owner route, correction, and recheck together. It prepares an owner review; it does not certify readiness or approve the work.

This log does not grant access, expand scope, approve work, send customer messages, change records, move money, or authorize financial, legal, security, privacy, employment, clinical, regulated, or policy decisions. Authorized owners control those decisions.

Name one narrow task, its training stage, and the result a reviewer can check. Do not treat a completed record as permission to repeat or expand the work.

Link the approved SOP, example, and safe evidence location. Do not paste credentials, customer or employee data, payment details, private attachments, restricted reports, or system exports into this log.

Reviewer finding and correction

Record what matched, one specific miss or unanswered question, and the rule or example to correct. A reviewer can compare a bounded sample and route a question; they do not grant access or approve independence.

Held action, owner route, and recheck

Name the authorized decision owner, held action, stop condition, and business-set recheck. Access, scope, customer messages, money, record changes, and exceptions stay with authorized owners.

Review steps

Prepare the record. Stop before treating evidence as authorization.

A preparer can organize approved references and flag a mismatch. The authorized owner decides what happens next.

  1. Start with one approved task, one checkable result, a named reviewer, and a safe SOP or example reference. Hold the record if any of those basics is missing or unclear.
  2. Record the training stage and a safe reference to the observed, practiced, or checked sample. Do not copy credentials, personal data, payment details, private attachments, restricted reports, or system exports.
  3. Compare the bounded sample with the approved rule or example. Record one specific match, miss, question, or correction instead of calling the work ready or not ready.
  4. Keep access, scope expansion, customer communication, money, record changes, exceptions, and business decisions held for the named authorized owner. A preparer or reviewer can organize evidence and route a question, but cannot authorize those actions.
  5. Record the owner decision reference and a business-set recheck after a correction, process, system, access, scope, reviewer, or policy change.

A completed training record does not grant authority.

Use the written escalation route when the source, task, evidence, reviewer, owner, access boundary, or next decision is unclear.

  • The task has no approved SOP or example, safe evidence reference, named reviewer, authorized owner, or clear checkable result.
  • A training record is being used to grant access, expand scope, approve work, send a customer message, change a record, move money, or make an exception.
  • The evidence includes credentials, customer or employee data, payment details, private attachments, restricted reports, or system exports.
  • The task, system, access, scope, reviewer, or policy changed but the training record still relies on an old source or example.
